Item Perceived quality of life and meanings attributed to the experience of extreme maternal morbidity: a qualitative study(Associacao Brasileira de Pos - Graduacao em Saude Coletiva, 2022) Narváez, Natalia Chávez; Acosta Ramírez, NaydúExperiencing extreme maternal morbidity (EMM) implies multiple changes, where the perception of quality of life is modified, generating a gap between what is perceived and what is desired. The aim is to understand the meanings attributed to EMM by survivors of the experience and the effect of these meanings on the processes of reconstruction of daily life. A qualitative study from the perspective of symbolic interactionism that included an analysis based on elements of grounded theory by conducting semi-structured interviews with women from a Colombian city. The experience of EMM is understood by the participants as “a suffering that is worthwhile” and is constructed from the meanings given to each stage that is experienced: (1) pregnancy: an unexpected event that is better when accompanied by a partner/family; (2) the complication: separation distress; (3) life after near-death. Women’s perception of quality of life depends on both the experience of the event and social support, as well as on the baby’s survival. Extreme Maternal Morbidity (MME) is a public health problem that affects both population health indicators, as well as at the social, family and individual levels, since it implies a situation of multiple changes for the women affected, where their perception of well-being and quality of life is modified by generating a gap between what is perceived and what is desired, mediated by the meaning that the woman gives to her experience, parallel to the social support and configuration of institutional actions in the health sector. Goals. Understand the meanings that MME survivors construct around their experience and the influence of these in their recovery and reconstruction processes of their daily lives in a Colombian city. Methods. Qualitative approach with the approach of symbolic interactionism and analysis with elements of grounded theory to approach the understanding of the phenomenon, using in-depth interviews and support in field notes. Results. The meanings that women give to the experience are grouped into 4 categories: meanings of the MME, meanings of survival, mechanisms of coping and reconstruction of daily life and an emerging category related to the perception of care services. Conclusions. The perception of quality of life depends not only on the experience of MME but also on family and partner support, in addition to the survival of the baby. In the latter case, there is a greater impact on the perception of quality of life of women. The distant and sometimes traumatic relationship with health personnel generates negative effects on women, accentuating feelings of stress, worry and loneliness. The role of health services in comprehensive and humanized care is fundamental.Item Perceptions of adolescent pregnancy in the rural context and the Colombian armed conflict: a qualitative approach based on social determination of health(2021-12) Palacios Perdomo, Herlan; Acosta Ramírez, NaydúBackground: Adolescent pregnancy in rural areas is a persistent health problem that has still not properly been understood. Studies with qualitative perspectives that address this phenomenon as a complex social process, which involves the recognition of the voices of the actors involved and the analysis of the specific context in which it takes place, are limited. Objective: This research explored the perceptions of young people and other social actors (municipalities of Palmira and El Cerrito in Colombia) of the social forces and dimensions of the social determination of adolescent pregnancy in the Amaime river basin. These geographic areas have been scenes of armed violence with various groups in combat within the context of a long-standing political conflict in Colombia. After the 2016 Havana agreements were signed, peacebuilding has been underway in its territories. Methods: A qualitative study that implemented focus groups and semi-structured interviews was conducted. The theoretical approach of social determination of health proposed by Breilh was used to study the social process entailed in adolescent pregnancy. Perceptions about social conditions, specific ways of life, and lifestyles were addressed. Galtung and Fischer’s theoretical approach on violence and peacebuilding was also incorporated to enrich the understanding of the Colombian context. The analysis was conducted with approaches from phenomenology. Results: Living conditions with strong social stigma and demand for social, political, and cultural opportunities were found. Regarding ways of life, little communication and information about sex education was perceived. As for lifestyles, there are youthful behaviors infused by sociocultural traditions that affect life projects and sexual behavior. Gender relations are precarious, and there are various types of violence that limit effective peacebuilding. Conclusions: This study contributes to a priority issue in sexual and reproductive health, with an approach that generates analytical elements to comprehensively expand the social and health interventions required.Item Youth Participation in the Prevention of the Consumption of Psychoactive Substances. A Case Study across Healthy Schools, Cali-Colombia(Universidad del Rosario, 2023-09-01) de los Ríos, Reina Myriam Villegas; Acosta Ramírez, NaydúIntroduction: the use of psychoactive substances (spa) in schoolchildren is a complex phenomenon with multiple negative effects. From the perspective of social determinants and health promotion, the educational institution (ie) is a decisive agent for participatory actions. The objective was to identify individual and institutional factors associated with the participation of students in a public school in Cali (Colombia) to prevent spa consumption. Materials and methods: case study with a mixed approach. The qualitative approach involved a documentary analysis to explore the response and institutional factors of the school selected as a critical case; while a quantitative component was developed to identify the relevant individual factors by means of a survey of 156 students (high school-middle school) of that school. Results: students identified peer or friend pressure as a relevant factor for spa consumption; it is evident that the most important prevention spaces and processes are communication with peers. The curricular work from the areas to strengthen decision making is also recognized as an institutional factor. Conclusion: in the healthy schools strategy, the alliance between the educational sector and the health sector is crucial in order to promote spaces and participation processes aimed at preventing spa consumption.
