dc.description	Se presenta un caso de un paciente adulto mayor con una úlcera arterial crónica atribuida a una escleroderma (SSc), sin embargo, se evidencian múltiples alteraciones sistémicas explicadas por un fenómeno de poliautoinmunidad. En esta condición se combinan diversas y complejas alteraciones de la inmunidad que generan síndromes clínicos de difícil interpretación causando confusión y retrasos en el diagnóstico y manejo. Este paciente, atendido en un hospital de nivel I y con antecedente de enfermedad de Hansen, exhibe un compromiso inicial mucocutáneo y posteriormente con hallazgos a nivel hematológico,pulmonar y de autoinmunidad que sugieren una enfermedad sistémica difícil de diferenciar entre una SSc y el Lupus Eritematoso Sistémico (LES). Ello generó retrasos en el inicio del tratamiento farmacológico inmunosupresor y gran discapacidad en el paciente, así como estancias prolongadas de hospitalización. Se evidencian fallas en el enfoque integral de pacientes con una condición autoinmune y barreras de acceso para la atención especializada en instituciones de alta complejidad. Este reporte intenta ratificar la teoría acerca de la presencia de poliautoinmunidad en una proporción significativa de pacientes con esclerodermia en la población afrolatina.
dc.description.abstract	A case of an elderly patient with a chronic arterial ulcer attributed to scleroderma (SS) is presented; however, multiple systemic alterations explained by a phenomenon of polyautoimmunity are evident. In this condition, various and complex alterations of immunity are combined that generate clinical syndromes that are difficult to interpret, causing confusion and delays in diagnosis and management. This patient, treated at a level I hospital and with a history of Hansen disease, exhibits initial mucocutaneous involvement and later hematological, pulmonary, and autoimmune findings that suggest a systemic disease that is difficult to differentiate between SSc and Systemic Lupus Erythematosus (SLE). This generated delays in the start of immunosuppressive pharmacological treatment and great disability in the patient, as well as prolonged hospitalization stays. Flaws are evident in the comprehensive approach to patients with an autoimmune condition and access barriers to specialized care in highly complex institutions. This report attempts to ratify the theory about the presence of polyautoimmunity in a significant proportion of patients with scleroderma in the Afro-Latino population.
