| dc.description | Se presenta un paciente masculino de 69 años con antecedentes de enfermedad renal crónica y diabetes mellitus tipo 2, que ingresó al servicio de Urgencias y luego a la UCI con diagnóstico de sepsis de tejidos blandos secundaria a gangrena de Fournier. El paciente había presentado un cuadro clínico de un mes de evolución con hemorroides, pero durante el examen físico se evidenció necrosis en el periné y el ano, con signos claros de infección. Debido a la gravedad de su condición, se le realizó una colostomía y desbridamiento perineal el mismo día de su ingreso, posterior a esto se ingreso paciente en UCI, donde enfrentó complicaciones severas como inestabilidad hemodinámica, insuficiencia respiratoria aguda, choque séptico e insuficiencia renal crónica agudizada. A pesar del tratamiento intensivo, incluyendo soporte vasoactivo, ventilación mecánica y terapia de reemplazo renal, la condición del paciente continuó deteriorándose. Sufrió un paro cardiorrespiratorio y fue reanimado, pero su estado neurológico empeoró, y la familia decidió no continuar con las medidas de reanimación, lo que llevó al fallecimiento del paciente. De aquí la relevancia de este caso clínico, se puede ver reflejada la complejidad y severidad de esta patología, subrayando la importancia de un manejo temprano y agresivo en casos de gangrena de Fournier, así como las decisiones éticas involucradas en el manejo de casos de alta complejidad. | |
| dc.description.abstract | We present a 69-year-old male patient with a history of chronic kidney disease and type 2 diabetes mellitus, who was admitted to the Emergency Department and then to the ICU with a diagnosis of soft tissue sepsis secondary to Fournier's gangrene. The patient had presented with hemorrhoids for one month, but during the physical examination necrosis was evident in the perineum and anus, with clear signs of infection. Due to the severity of his condition, a colostomy and perineal debridement was performed on the same day of his admission. After this, the patient was admitted to the ICU, where he faced severe complications such as hemodynamic instability, acute respiratory failure, septic shock, and chronic renal failure. sharpened. Despite intensive treatment, including vasoactive support, mechanical ventilation, and renal replacement therapy, the patient's condition continued to deteriorate. He suffered cardiorespiratory arrest and was resuscitated, but his neurological condition worsened, and the family decided not to continue with resuscitative measures, which led to the patient's death. Hence the relevance of this clinical case, the complexity and severity of this pathology can be seen, underlining the importance of early and aggressive management in cases of Fournier's gangrene, as well as the ethical decisions involved in the management of high-risk cases. complexity. | |
